Tufts Medical Center
Tufts Medical Center is the Boston academic medical center renowned for its highly complex care, its collaboration with community providers, and its commitment to high-quality, lower cost health care. In 2015, Tufts MC was named one of only 13 Five-Star academic medical centers nationally, and the only one in New England. Tufts MC includes an adult hospital and children’s hospital. Tufts MC is the principal adult and pediatric teaching hospital for Tufts University School of Medicine and is among the top 10 percent of all institutions receiving National Institutes of health (NIH) funding.
MelroseWakefield, formerly Hallmark Heath, is a coordinated system of hospitals, physician practices and community-based services providing care for communities throughout north suburban Boston. Since its inception in 1997, MelroseWakefield has been committed to its mission of providing quality care for its communities and achieving clinical excellence for the patients it serves. The system includes MelroseWakefield Hospital, Lawrence Memorial Hospital of Medford; Hallmark Health Medical Center, Reading; Hallmark Health Medical Associates; Hallmark Health VNA and Hospice; and Lawrence Memorial/Regis College School of Nursing.
Lowell General Hospital
Lowell General Hospital is a regional health care provider with two hospital campuses and multiple care sites throughout the Merrimack Valley. Lowell General Hospital has a long-standing and proud history of providing high quality care at a lower cost compared to many other hospitals in Massachusetts. The hospital has been awarded magnet status by the American Nurses Crediential Center multiple times, making it one of only 4 percent of hospitals nationwide. Lowell General has also been recognized as a Top 100 Community Value Provider by Cleverley + Associates. For seven years in a row, the hospital has been named a Best Place To Work by the Boston Business Journal.
Floating Hospital for Children
Floating Hospital for Children is an exceptional, full-service pediatric hospital. The main hospital is located in Boston and affiliate sites serve patients in Lowell, Lawrence, Brockton, MetroWest, Westford and Cape Cod. Children receive care in every specialty from general pediatric services to the care of the most complex cancers. Our pediatric specialists are among the most talented and highly trained professionals in the country.
Home Health Foundation
Home Health Foundation, which includes Home Health VNA, Merrimack Valley Hospice and HomeCare, Inc., is one of the most respected home health and hospice agencies in this region. Home Health Foundation is a multi-state, full-service provider of certified home health, palliative, hospice and private duty services with annual revenue of $60 million. The agency services 110 cities and towns throughout Northeastern Massachusetts and Southern New Hampshire with five offices and a 21-suite hospice house which together provide care to more than 16,000 patients annually. Additionally, Merrimack Valley Hospice serves the Southern Maine region as York Hospital Hospice in partnership with York Hospital. The agencies play a critical role in the health and welfare of patients and families, and are committed to innovation, clinical excellence and personalized care.
Lowell General Physician Hospital Organization
The Lowell General PHO is 355 member physicians strong. The PHO was founded in partnership with Lowell General Hospital in 1995 with the goal of developing a local, integrated delivery system providing outstanding quality care to patients in the Greater Lowell area. Our caregivers share the vision to create a healthcare environment that demands quality, promotes wellness, improves outcomes, eliminates errors, enhances communication and creates a full spectrum of healthcare value to its patients.
New England Quality Care Alliance
NEQCA is more than 1,800 physicians across eastern Massachusetts caring for more than 500,000 patients. Often referred to as the network where the doctor still matters, NEQCA is a partnership of community and academic physicians dedicated to providing comprehensive, innovative, high quality and affordable health care that brings value to patients and the community. Tufts Medical Center is NEQCA’s academic partner. Known for its high quality, NEQCA scores better than four out of the six major Massachusetts physicians networks on MHQP HEDIS scores. The Hallmark Health Physician Hospital Organization joined NEQCA in January 2017.
