Tufts Medical Center
Tufts Medical Center is internationally respected academic medical center located in Boston, Mass. The Medical Center is the home of an adult hospital and a full-service children’s hospital, Floating Hospital for Children. Tufts MC is the principal teaching hospital for Tufts University School of Medicine. Our research enterprise is among the top 10% of all institutions receiving National Institutes of Health(NIH) funding. Tufts MC is renowned for its highly complex care, its collaboration with community providers, and its commitment to high-quality, lower cost health care. Tufts MC was just named among the top 10 academic medical centers nationally for its quality and patient experience by Vizient and performs the most heart transplants in New England.
MelroseWakefield Healthcare, formerly Hallmark Health, is a coordinated system of hospitals, physician practices and community-based services providing care for communities throughout north suburban Boston. Since its inception in 1997, MelroseWakefield Healthcare has been committed to its mission of providing quality care for its communities and achieving clinical excellence for the patients it serves. The system includes MelroseWakefield Hospital, Lawrence Memorial Hospital of Medford; Hallmark Health Medical Center, Reading; Hallmark Health Medical Associates; Hallmark Health VNA and Hospice; and Lawrence Memorial/Regis College School of Nursing.
Lowell General Hospital
Lowell General Hospital is a regional health care provider with two inpatient campuses and multiple care sites throughout the Merrimack Valley. Lowell General Hospital has a long-standing and proud history of providing high quality care at a lower cost compared to many other hospitals in Massachusetts. Lowell General Hospital is consistently recognized both regionally and nationally for its high quality care and its work environment. The hospital has been awarded Magnet status by the American Nurses Credentialing Center twice, making it one of only nine percent of Magnet-recognized hospitals nationwide. Floating Hospital for Children
Floating Hospital for Children is an exceptional, full-service pediatric hospital. The main hospital is located in Boston and affiliate sites include Lowell, Lawrence, Brockton, MetroWest, Westford and Cape Cod. Children receive care in every specialty from general pediatric services to the care of the most complex cancers. Our pediatric specialists are among the most talented and highly trained professionals in the country.
Home Health Foundation
Home Health Foundation, which includes Home Health VNA, Merrimack Valley Hospice and HomeCare, Inc., is one of the most respected home health and hospice agencies in this region. As the fourth member of Wellforce, Home Health Foundation is working to integrate Circle Home and Hallmark Health VNA and Hospice into its continuum. Home Health Foundation is a multi-state, full-service provider of certified home health, palliative, hospice and private duty services with annual revenue of $60 million. The agency services more than 110 cities and towns throughout Northeastern Massachusetts and Southern New Hampshire with five offices and a 21-suite hospice house which together provide care to more than 16,000 patients annually. Additionally, Merrimack Valley Hospice serves the Southern Maine region as York Hospital Hospice in partnership with York Hospital.
Lowell General Physician Hospital Organization
The Lowell General PHO is approximately 400 physicians strong. The PHO was founded in partnership with Lowell General Hospital in 1995 with the goal of developing a local, integrated delivery system providing outstanding quality care to patients in the Greater Lowell area. Their caregivers share the vision to create a healthcare environment that demands quality, promotes wellness, improves outcomes, eliminates errors, enhances communication and creates a full spectrum of healthcare value to its patients.
New England Quality Care Alliance
Founded in 2005 by Tufts Medical Center and Primary Care LLC, NEQCA is a partnership of community and academic physicians dedicated to providing comprehensive, innovative, high quality and affordable health care that brings value to their patients and communities. In addition to negotiating and managing highly competitive commercial and government payer contracts on behalf of these physicians, NEQCA helps practices manage change by developing and implementing programs that make it easier for physicians to succeed in a rapidly changing industry. NEQCA’s more than 1,700 affiliated physicians proudly refer patients to their colleagues at Tufts Medical Center and Floating Hospital for Children when complex care is required.
