Advancing Accountable Care

    As Wellforce ACO partners, our group of hospitals and providers are working closely together to coordinate MassHealth patients’ care and connect them with available services and supports for all of their health care needs. This coordination helps the patient get the right care at the right time, in the right place. 

    To prepare for its launch, a team from Fallon Health and our member organizations were busy creating educational and promotional materials and webinars to reach both providers and prospective members. The plan now includes 52,387 members and 1,893 providers. Here is a quick snapshot of how it all came together:

    • 72 Trainings/educational webinars provided to Wellforce Care Plan (WCP) providers

    • 34 Weekly WCP email updates to WCP providers

    • 167 Q&As in WCP Provider FAQ document

    • 12 provider-facing and 15 member/prospect materials created

    • 26,300 flyers printed in English and 12,550 flyers in 9 different languages

    But Wellforce Care Plan is about so much more than the numbers. It is about the people our teams are now able to help. Take the example of a 12-year-old boy with uncontrolled Type 1 diabetes. The child had gone through some challenging times due to his mother’s battle with addiction. He was struggling to accept his health condition. A Wellforce Care Plan nurse visited his home and began a thoughtful process to help him heal. She did some research and found a camp for children with diabetes in Durham, NH. Working with Floating Hospital for Children and the camp, the nurse was able to get the child’s camp tuition fully covered so he could attend. This is one example of how Wellforce Care Plan is helping patients and families live healthier lives with coordinated comprehensive care, not just medications and procedures.
